A.G. Gaston Jaguars Cheerleading Registration

BY COMPLETING THE ONLINE REGISTRATION FORM, YOU AGREE THAT YOU HAVE READ THE FOLLOWING STATEMENTS AND AGREE TO COMPLY WITH AND ABIDE BY THEM.

I hereby give my approval for my child’s participation in any and all cheer activities during the upcoming season. I assume all risks and hazards incidental to such participation including the transportation to and from the activities and do hereby waive, release, absolve, indemnify and agree to hold harmless the A.G. Gaston Boys & Girls Club, the organizers, the sponsors, the supervisors, participants and persons transporting my child except to the extent or limit and in the amount covered by accident or liability insurance. I also attest that my child has no medical condition that prohibits him/her from participating in cheerleading.

All equipment issued by A.G. Gaston Boys & Girls Club must be returned at the end of the season. Parents/guardians are liable for the replacement cost of all issued equipment other than normal wear and tear. Uniforms are the property of the player and must be maintained and worn in all games. Uniform is not to be worn at practice.

Registration fee must be paid in full prior to player being allowed to participate in any A.G. Gaston Cheer activities or receiving any A.G. Gaston Cheer issued equipment and uniform. NO REFUNDS WILL BE GIVEN.
